Purpose:
The MDS Coordinator supports the DON with staff management and on-call duties. They keep families informed about resident changes and physician visits, review wound care documentation, and assist with quality reports. Ensures care plans are complete, review charting and restorative programs to ensure compliance.
Essential Duties:
+ Conducts audits of ALF and SNF as directed by the Director of Nursing. Monitor resident charts for accuracy, i.e. falls, weight loss, and/or significant changes in resident condition; and ensure all resident vaccinations are current, consents are signed and documentation filed in medical record for the PPD (admission and annual), Influenza and Pneumococcal vaccines.
+ Assist the DON with QA reports to include the 802 and Casper Reporting as well as infections, Incidents, Wounds and Restorative Programs.
+ Assure that all medical needs of the residents are met and that a complete resident care plan is developed after determining completion of the MDS assessment by all responsible parties within regulatory time frames and complete MDS assessments and submissions as scheduled; attend weekly IDT meetings and participate in Resident Care Plan Conferences.
+ Assist in the development of nursing care plans and procedures for SNF and ALF. Maintain current knowledge of, and assure facility compliance with applicable State and Federal Laws and Regulations, as well as corporate standards; and assume responsibility in coordination with DON and Health Care Administrator for facility’s medical records and their compliance with applicable regulations.
+ All duties as assigned.
Required Qualification:
+ Two years of geriatric experience
+ current license as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the State of Florida.
+ Certification in CPR/AED and First Aid
+ Certification and one year experience in Minimum Data Set (MDS) Assessments.
+ For purposes of effective communication with residents, families and staff must be able to see, hear, speak, read and write English.
+ Maintain ability to stretch, push, pull, kneel, reach, twist, lift unassisted at least 50 lbs and walk or stand up to eight hours.
