Responsibilities for a MDS Coordinator with Phoebe Allentown:

Responsible for assessment scheduling and completion of the MDS in accordance with the RAI guidelines and with all practices set forth by the Pennsylvania Department of Health and Federal CMS.Oversees short term rehab skilled residents and coordinates with therapy and insurance companies as needed.Monitors and completes all required processes in accordance with the Federal Medicare and PA Medicaid Guidelines. All MDS Coordinators will operate in accordance with the practice parameters as set forth in the State Nurse Practice Act according to their licensure.Coordinates and schedules care conference meetings per regulationsReview and evaluate QM’s and QRP reports and attend meetings to discussParticipates in triple check process.    
Requirements for a MDS Coordinator with Phoebe Allentown:Education: Graduate of an approved school of nursing and currently licensed in Pennsylvania to practice as a Registered/ Licensed NurseExperience: MDS Coordinator short term rehab experience is preferredSchedule: Monday to Friday, NO WEEKENDS NO HOLIDAYS.
