Age Specific Responsibility

Must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served in assigned department.  Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient’s status and interpret the appropriate information needed to identify each patient’s requirements relative to age-specific needs and to provide the care needed as described in the department’s policies and procedures.

Summary                                                                                                                                          

Nurses at Mary Free Bed are a key member of the multi-disciplinary team that collaborates together to provide patient-centered care.  Under the direction of the Director of Nursing or Designee, the MDS Nurse will complete the Minimum Data Set (MDS) assessment for all patients/residents in accordance with the requirements of this state and the policies and goals of this facility.  The MDS Nurse will provide patient care within the scope of their license, certification or registration.  Nursing care is performed with compassion and courtesy, and each nurse is responsible to follow departmental policies and service standards. 

Essential Job Responsibilities                                                                   

Staff will provide patient care, treatment, and services within the scope of their license, certification or registration as required by law and regulation.Conduct and coordinate the development and completion of the Minimum Data Set (MDS) resident assessment in accordance with current rules, regulations, and guidelines that govern the resident assessment, inducing the implementation of RAP’s/CAA and Triggers/CAT’sConduct or coordinate the interviewing of each resident for the resident’s assessment.  Evaluate each resident’s condition and pertinent medical data to determine any need for special assessments or activities.Develop preliminary and comprehensive assessments of the nursing needs of each resident, utilizing the forms required by current rules or regulation and facility goals and policy.  Evaluates effectiveness of nursing care plans, and initiate updates to care plans as needed.Coordinate with the interdisciplinary team the development of a written plan of care (preliminary and comprehensive) for each resident that identifies the problems/needs of the resident, indicates the care to be given and goals to be accomplished.Ensure that the initial resident assessment is completed within appropriate time frame of the resident’s admission and locked and electronically submitted timely.Identify opportunities with documentation requirements and provide education to nurses and interdisciplinary team that strengthen documentation for improved patient care and reimbursement.Analyzes assessment data independently for complex patient problems and identifies early subtle changes that formulate PDPM categories and effect patient care and/or reimbursementAssists as the facility Medicare and/or managed care insurance utilization coordinator.  Advocate for insurance authorization requests that align with patient and facility goals. Fulfill responsibilities in Nursing Leadership on-call rotation as assigned.  Assist Nurse Leaders with daily care conferences, discharge planning and clinical follow-up as needed.May be required to work beyond normal working hours, on weekends and other positions temporarily when necessary and may be subject to call back during emergency conditions or State/CMS survey activity.  May be required to work shifts other than those for which normally assigned.Complete other duties as assigned to support all operations of the sub-acute rehab unit.Embrace Diversity, Equity and Inclusion, by:

Essential Job Qualifications                                                                                                          

Graduate of an accredited School of Nursing, RN preferred.  LPN will be considered with previous MDS experience.Valid nursing license in the state of Michigan.Valid CPR Certification.Minimum 2 years work experience as a nurse.Minimum 2 years work experience in a relevant clinical rehabilitation setting.Knowledge of CMS Resident Assessment Instrument (RAI) Manual preferred.
