Description:Position Title: Medical Assistant
Department: Diabetes Chronic Care
Purpose: The Medical Assistant (MA) supports the comprehensive care of patients in the Diabetes Chronic Care Management (DM-CCM) and Genesis Foodplex programs. The MA will collaborate with a multidisciplinary team, including Nurses, a Pharmacist and a Social Worker. Key responsibilities include providing operational and clinical support within the clinic; engaging with patients to assess social needs and connect them with community resources; monitoring patients, collecting health information and vital signs, and assisting with continuous glucose monitor placement and data collection; and completing prior authorization requests and facilitating new patient enrollment.
In the Genesis Foodplex program, the MA will coordinate food ordering, delivery, inventory management and distribution for participants facing food insecurity. Responsibilities include assisting with new participant enrollment, conducting food distribution clinics, and recruiting volunteers. They wil also support enrollments in Federal, state and local resource programs to enhance access to healthcare and social services.

Supervisory Responsibility: No Supervision: The job does not require the provision of guidance or supervision to others. There is no formal responsibility for directing others.
Materials Responsibility: Limited: Work requires limited responsibility for material resources. Examples of resources could include supplies, equipment, inventories, small budgets, and other similar material assets. The employee has a limited amount of control over these resources. The cost of errors is also limited in terms of damage, waste or financial loss. Problems associated with material resources are not complex. The volume of resources may vary, but the level of difficulty in dealing with these resources is uncomplicated.

POSITION SPECIFICATION
Education: High school plus additional course work up to 1 year
Field Of Study: Medical Assistant
Special Training: Basic Life Support; must complete Community Health Worker Training course within six months of hire date.
Training Preferred: ICD, CPT, Best Practices for Immunizations
Licensure/Registration: Certified or Registered Medical Assistant within 12 months.
Experience: More than 1 year experience required.
Interpersonal Skills: Interaction is with a variety of people inside or outside the organization. Communications are of moderate difficulty and sensitivity. Contact with others may involve detailed & lengthy dialogues & exchanges of information. Requires a moderate amount of interpersonal skills. Interactions involve dealing with moderately complicated problem situations or stressful encounters.
Working Conditions: There is limited exposure to moderately adverse and undesirable environmental conditions. There are some health and safety risks. Position may require safety equipment and precautions. The amount of time the employee may experience these moderate conditions is limited to 35% or less of their work day.
