ResponsibilitiesThis position is a per diem role and requires you to work at different clinics located in Queens, Bronx and Brooklyn.
Schedule: Monday-Friday 9:00-5:00 alternate schedule to accommodate evening hours may be needed
Obtain and document vital signs including; head circumference, pulse, respiration rate, height and weight, blood pressure, temperature; collect specimens for various labs including performing venipuncture; perform vision and hearing screening.Assist MD and/or PNP in provision of physical examinationsEnsure the medical records contain written documentation of all health and mental health services received.Follow up on appointments and referrals for specialists for a specified caseloadCompletion of camp and school medical forms for specified caseloadsTrack and monitor required health visits for specified caseloadsData entry of health information into electronic systems as requiredEnsure that documentation of recommended health visits that are obtained in community based settings are received and stored into medical record of specified caseloadAssist in quality assurance activities involving services provided to children in assigned caseloadAttend case conferences and Clinical Planning Conferences as assignedAttend Service Plan Review meetings when needed to provide health related information, which is germane to developing and implementing permanency plan for children in assigned caseload.Salary Range: $45,000 to $52,300/year

2 years clinical experience in pediatric clinic or physicians officeComputer skillsKnowledge of or experience in foster careKnowledge of and/or experience in management of medical recordsKnowledge of and/or experience in health care case managementCPR CertificationGood written and verbal communication skillsCertified to perform pre and post HIV test counselingSpanish speaking a plus Demonstrated strong commitment to safety.
