Summary:
This full-time position performs a vital role by delivering high-quality HIV Navigation Services to individuals living with HIV and those at a high risk of acquiring HIV. This role may accompany clients to their medical appointments at the REACCH Program, and/or other community agencies. This individual must have excellent attention to detail skills and be able to fully assess situations and prioritize follow-up. The expected caseload for this position is up to 60 clients for HIV Navigation Services. Please note, this staff member must have the ability to work afternoon and evening hours at the REACCH Program 1 evenings per week (until 6:00 PM). This role will include performing rapid HIV testing and linking individuals to the REACCH Program’s pre-exposure prophylaxis (PrEP) and post-exposure prophylaxis (PEP) services. Ample training and shadowing will be provided by the REACCH Program, the Pennsylvania Department of Health, and the program’s fiscal intermediary. This position will report to the REACCH Program’s Case Management and Social Work Supervisor. Supervision will be provided for a minimum of one hour per week.
Primary Duties – HIV Services Navigation (HNS) and HIV Testing:
Provide HIV Navigation Services to approximately 50-60 clients in the form of pre-exposure prophylaxis (PrEP) and post-exposure prophylaxis (PEP) referral and linkage, and other risk-reduction activities. Prompt follow-up with clients and referral agencies to ensure clients have access to PrEP or PEP medications and encourage adherence. Serve as a resource to clients for community resources and referrals. Serve as a liaison and advocate between clients and insurance companies to support navigation of insurance coverage, co-pay assistance programs, and insurance appeals. Co-facilitate community outreach events for education and HIV testing with REACCH team members. Track HIV testing outcomes, community outreach deliverables, etc. Complete required EPIC, CAREWare, and SNNLP documentation requirements within 5 business days of interaction.Secondary Duties:
Carry a small caseload (up to 10) of Anti-retroviral Treatment and Access of Services (ARTAS) clients – provide short term case management and support surrounding new diagnosis and linkage to HIV care. Complete outreach to lost-to-care clients in emergency departments, client homes, and the community, as needed. Complete new patient social work assessments and acute coverage for people living with HIV when needed. Complete monthly SNNLP and HNS Dept of Health reporting when needed. Complete other tasks as assigned.
