Position Purpose:
Join Mercy Care Atlanta as a Medical Case Manager, where you'll deliver patient-centered care and provide leadership to multidisciplinary teams. This advanced role focuses on supporting individuals living with HIV and/or HCV through care coordination, medical adherence, and resource connection to improve health outcomes.
What You Will Do:
Coordinate care across Behavioral Health, Substance Use, Dental, Social Determinants of Health, and other specialties.
Support adherence programs like STOPGap medication and long-acting injectables (e.g., Cabenuva).
Develop personalized care plans and monitor medical progress, including viral load suppression and routine screenings.
Refer patients to internal and external resources such as behavioral health, oral health services, or peer support.
Maintain a caseload of up to 90 patients requiring intensive care coordination.
Assist with quality improvement initiatives and Ryan White quarterly progress reports.
Represent Mercy Care at community events and health fairs (3-4 annually).
Minimum Qualifications:
Bachelor’s degree required; BSN or equivalent experience preferred.
3-5 years’ experience with target populations or lived experience.
1 year of experience in HIV care preferred.
Advanced knowledge of Ryan White programs, quality improvement processes, and community resources.
