As a Medical Chart Auditor, you will:
+ Perform detailed reviews of patient medical records to ensure documentation accuracy, completeness, and compliance with coding standards (e.g., ICD-10, CPT).
+ Validate appropriateness of diagnoses, procedures, and other elements for proper billing and reimbursement.
+ Identify any inconsistencies or gaps in chart documentation and communicate findings to providers, coders, or department leaders.
+ Create detailed audit reports summarizing findings, trends, and recommendations for process improvement.
+ Monitor compliance with federal, state, and accreditation bodies such as Medicare, Joint Commission, or other governing agencies.
+ Conduct training for staff, including providers and coders, on quality documentation practices and regulatory updates.
+ Collaborate with interdisciplinary teams to implement corrective action plans when deficiencies are identified.
Requirements
+ Minimum of 3 years experience in health information management, chart auditing, or medical coding within a hospital or clinical setting preferred.
+ Strong understanding of clinical documentation practices, coding guidelines, and regulatory/insurance requirements.
+ Exceptional attention to detail and accuracy.
+ Strong analytical skills with the ability to interpret complex medical records.
+ Proficient in medical terminology, anatomy, and physiology.
+ Advanced computer and EHR proficiency (e.g., Epic, Cerner, or similar systems).
+ Ability to communicate findings effectively to diverse audiences, including providers and administrators.
