**Experience & Requirements** + High School graduate or equivalent. + Minimum one-year medical records experience. + Familiarity with the Problem Oriented Medical Record (SOAP) system essential. _A background check, security clearance and drug screening are required for this role._ **Responsibilities** Below is a list of your responsibilities as a **Medical Records Clerk** with YesCare at the Philadelphia Detention Center, 8151 State Road, Philadelphia, PA 19136 + This is an Evening Shift 3-11:30 pm; Alternating Weekends + Starting rate $18.87 with shift differentials. + Ensures confidentiality of all medical records information, and completes forms for release of information; utilizes electronic medical record system eCW as applicable throughout the scope of duties and responsibilities + Retrieve medical charts for all healthcare staff or clinicians as requested. File daily all in use medical records. Secure all active and inactive medical records. + Assure that charts are counter-signed by Physician, check charts for completeness. + Release information at the direction of the Medical Records Supervisor, Medical Director or Health Services Administrator. + Answer the telephone, take messages and make telephone calls.
