Medical Records Specialist - Milwaukie, OR Job Description + Maintain a sound medical record, ensuring all components are appropriately filed, including, but not limited to: admission paperwork, correspondence, financial, and medical documents. + Thin medical records periodically according to policy and maintain inventory of thinned files. + Purge and rotate records annually according to agency, State, and Federal retention schedules. + Within established agency and regulatory standards, assist with the maintenance of patient charts, including assembly, filing, and assistance with purging and thinning. + Assist agency personnel with electronic medical record creation and the closing of episodes or patient charts. + Complete monthly Quality Improvement report from Intake department ensuring accuracy. Audit chart and report missing paperwork, last contact, and discharge documentation. + Update documentation in chart as directed. + Assist new support staff in the state data collection software, Measures and Outcomes Tracking System (MOTS) and archive databases. + Assist clinicians with MOTS such that assessment and discharge forms are completed according to MOTS standards; evaluate for errors and omissions, report and notify supervisors and counselors of errors. + Make administrative edits. + Follow up as necessary. Interface with clinic staff, patients, and visitors in a professional manner. + Demonstrate effective interpersonal communication skills which promote cooperation and teamwork. + Assist with receiving and routing a high volume of calls on multi-line telephone system; provide basic service information; schedule appointments for Portland Recovery Center. + Assist with preparing charts for internal (quality) and external (JC, State, County, etc.) audits, reviewing records for completeness and accuracy; relay information for final chart audit. + Respond to internal and external medical records (both paper and electronic) requests in accordance with state and federal regulations. + Assist patients with admissions document completion ensuring accuracy. + Provide timely and accurate data entry into the electronic medical record in compliance with agency and regulatory standards. + Maintain the integrity of the electronic medical records system within individual scope of job duties. + Ensure forms are “clean” and most up to date version is available by maintaining business forms in document server in an organized manner. + Assist with maintaining office machines, reporting problems as necessary. + Maintain strict confidentiality regarding patient Protected Health Information and other related data consistent with all applicable laws and regulations. + Provide professional and high quality customer service. + Achieve outcomes using methods that align with the company's mission and values: Communicate accurately and positively about recovery and the use of evidence-based treatments. + Use data and science to identify barriers, overcome obstacles, and inform decisions. + Be financially responsible; avoid waste, document accurately, seek new opportunities to serve. + Demonstrate compassion and respect for others.
