ABOUT THE ROLL
The Nurse Care Manager at Bethlehem Haven's Medical Respite provides medical care to patients experiencing an acute recuperative need in a high-touch environment. The Respite Nurse Care Manager provides hospital consultation for possible respite admissions, functioning as the primary liaison to assess for clinical appropriateness for respite level of care. The Respite Nurse Care Manager will meet with hospital staff and patient prior to referral approval to ensure respite would best meet the patients need. Then send necessary clinical information to Pittsburgh Mercy Family Health Center MD for final approval. Ensure all post hospital follow up with home health, medications, medical equipment, appointments and all other needs are scheduled and accounted for prior to hospital discharge and transfer to respite. During a patient's respite stay the Respite Nurse Care Manager will make sure all medications are ordered and reconciled and confirm that all medical care is being follow up on, including home health and appointments with specialists. This position will complete ongoing monitoring of respite patients competition of treatment goals to identify readiness for discharge
RESPONSIBILTIES/REQUIREMENTS:
Current/Valid RN license in the State of PA is required with at least 2 years’ experience in a medical setting is required. Behavioral health experience is preferred.Current/Valid PA Drivers’ License is required.Must have excellent communication skills both written/oral along with great attention to detail relating to documentation, deadlines, and other deliverables.Must be able to obtain ACT 33/34/73 clearances upon hire.Schedule:
Monday – Friday 8a-4p. (This is a non-exempt hourly position)
