Medical Social Worker, Hospice

BAYADA Home Health Care is currently seeking an energetic and experienced **Licensed Medical Social Worker, MSW, for a full time opportunity** performing home health visits with our Fayetteville **Hospice** office. This office services clients on a per visit basis in territories throughout **Bladen, Cumberland, Harnett, Hoke, Moore, Robeson, and Sampson Counties** .
Prior hospice experience preferred, but not required. As a Medical Social Worker, you will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to clients, affording them the opportunity to receive the medical care required to remain at home.
**Responsibilities include:**
* Assessing emotional factors related to terminal illness.
* Assisting the physician and other IDG members in recognizing and understanding the social/mental stress and/or disorder that exacerbates the symptoms related to terminal illness.
* Assessing the patient/family psychosocial status.
* Assessing environmental resources and obstacles to maintaining safety.
* Participating in the development and revision of the plan of care.
* Preparing clinical and progress notes.
* Identifying family dynamics and communication patterns.
* Identifying and utilizing appropriate community resources and assessing client/family ability to
access them.
* Assessing caregiver’s ability to function adequately.
* Identifying support systems available to reduce stress and facilitate coping with end-of-life care.
* Evaluating for long-term care when appropriate and assessing ability to accept change in level of care.
* Evaluating for respite care when appropriate and coordinating respite stays.
* Assisting client/family in assessing financial resources when appropriate.
**Qualifications for the ideal candidate:**
***** Have a Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education; **OR**
As permitted by state regulation, have a baccalaureate degree from a school of social work accredited by the Council on Social Work Education, as indicated by school transcript or diploma, and is supervised by an MSW (or LCSW, if required by state regulation) **OR**
As permitted by State regulation, have a baccalaureate degree in social work, psychology, sociology, or other field related to social work, and is supervised by an MSW (or LCSW, if required by state regulation) **AND**
* Meet all state practice requirements, and
* Have a minimum of one year experience in social work, preferably hospice or palliative care, and
* Have a criminal background check completed
