Astria Health is seeking a Medical Social Worker to join our team! This will be a day shift position working Monday-Friday.

 

What you get to do every day!

 

Under the supervision of the Director of Case Management/Utilization Management, performs social work services and functions to patients of all ages in the area(s) of assignment, and assists in carrying out assigned tasks for the department. You will:

 

Continuously performs psychosocial assessment, counseling, and discharge planning with identified patients of all ages, including children, adolescents, adults, or geriatric patients. Frequent communication in person or by telephone with patients/families, staff, physicians, and community agencies. Ongoing development, implementation, and modification of care plans for discharge, based on ongoing assessment findings. Continuously educates patient/families regarding community resources and/or illness/injury process. Frequently provides crisis intervention through skillful use of counseling techniques, gathering  information for assessment and planning purposes, defusing the crisis of the moment, and consulting with physician regarding treatment options and their psychosocial impact.Occasionally performs data collection for department Quality Management ImprovementPerforms other related work as required.Collaborate effectively across organizational boundaries.Adheres to the core values and standards of the organization.

 

Bachelors of Social Work or Masters of Social Work. LICSW or LASW Washington State Licensure One or more years’ experience with adult learning principles to support nursing education and training activities. Experience in Case Management collaboration, crisis intervention, advocacy, treatment planning, cultural competence, and strong communication skills are essential. Self-directed, ability to work both independently and collaboratively with hospital community. Ability for public speaking and development/delivery of presentations. Demonstrates effective leadership and interpersonal skills. MS Office Suite skills, Internet and associated computer skills (intermediate skill level).

 

Required Qualifications and Education.

3 years’ experience in an Acute Care Hospital setting preferred.
