RESPONSIBILITIES I. JOB SUMMARY/RESPONSIBILITIES: • Provides services including short-term counseling, resources identification, information, and referral, develops and implements a plan of care requested by the physician which will enhance the patient’s capacity to function in their environment. • Performs duties in accordance with policies and procedures established by the agency. II. TYPICAL PHYSICAL DEMANDS: • Sitting, finger dexterity, seeing, hearing, speaking. • Constant: reaching above, at and below shoulders; repetitive arm/hand motion and frequent gripping of objects. • Frequent: standing, walking, stooping/bending, and twisting body. III. TYPICAL WORKING CONDITIONS: • Subjected to adverse environmental conditions like blood borne pathogens. IV. MINIMUM QUALIFICATIONS: A. EDUCATION/CERTIFICATION AND LICENSURE: • Current Hawaii State License as a Licensed Social Worker. • Master's degree in social work. • Valid U.S. driver’s license and vehicle with insurance as required by Hawaii State Law. • Driver’s abstract or evidence of good driving record. • Current BLS certification. B. EXPERIENCE: • Minimum one (1) year experience in a health care setting. • Prior Home Health experience. • Ability to communicate in English effectively both orally and in writing. • Ability to establish and maintain effective working relationships with clients, staff, and the public. • Knowledge of professional medial social work theory and practice to implement and evaluate patient care.
