Medical Social Worker I

 

Provides social work services to patients, families and identified caregivers as part of an interdisciplinary care team.


This includes conducting psychosocial assessment, intervention and counseling with a particular emphasis on transitions of care in a healthcare setting
Maintains current knowledge of resources available and shares this information with patients, families and identified caregivers
Collaborates across the organization and with community stakeholders
Works under broad supervision
Performs other related duties as assigned
Familiar with standard concepts, practices, and procedures within the field
Regular and predictable attendance is an essential job function
Competent to meet age specific needs of the unit assigned

  PRN (As Needed) -Day Shift     

Requirements and Minimum Qualifications:


Master’s degree in Social Work required
Licensure and/or certification as required by the State of Idaho; Idaho license by endorsement must be initiated by time of hire and obtained within 3 testing cycles, not to exceed 1 year of employment
1 year of social service experience in a health care setting or social services agency preferred
Knowledge of and ability to obtain community resources (information and referral, state/federal financial programs and eligibility requirements, etc.). Compliance with accepted professional standards and practices

 

Working Conditions:


Must be able to maintain a standing and/or sitting position
Typical equipment used in an office job
Repetitive movements
