We are a CA based hospice agency looking for a compassionate and dependable Hospice Social Worker to join our team. We are looking for individuals that understand that addressing needs on a mental and emotional level is just as important as alleviating pain and suffering on the physical one. Members of our support services team are leaders in navigating difficult end-of-life circumstances along with celebrating the lives of those we work with all while meeting a shared goal of helping individuals live their final days with dignity and comfort. We currently have needs for a Social Worker, part-time in our Contra Costa County division.

Although this position is Monday through Friday from 8:30 - 5pm, our Hospice Social Worker role is autonomous with built in flexibility based on your patients’ needs. ResponsibilitiesEssential Job Functions / Responsibilities:
Provide in-home and facility visits to assess patient psych-social status and needs while providing emotional support
Develop MSW care plan and institute appropriate interventions
Develop and collaborate with community resources and third party providers as needed
Educate and prepare patients, families and caregivers to end-of-life processes from a social work lens
Collaborate and communicate with referral partners as appropriate
Provide educational inservices to outside care providers as appropriate
Complete accurate and relevant clinical notes in EMR

Qualifications
Masters of Social Work
Experience working with death and dying providing support to patients, families and caregivers preferred
Licensed driver with automobile that is insured in accordance with state requirements
Maintains current CPR Certification that meets American heart Association BLS Guidelines
