Laredo Medical Center’s (LMC) 326 bed acute care hospital is currently looking to hire an Inpatient Rehabilitation Facility Patient Assessment Instrument (IRA-PAI) Coordinator to become part of our dynamic team of acute inpatient rehabilitation services in providing high quality, patient-centered care to those who have experienced a loss of function from an injury or illness.

LMC allows you to work in a collaborative environment with colleagues and leadership with exposure to a variety of patient care levels. Providing post-acute care services for our underserved community, we are looking to couple experienced and dedicated staff in providing high quality patient care and customer service to our patients in supporting them on their road to recovery. Position's Purpose and Essential Functions:

The IRF PAI Coordinator assumes the task to complete each IRF-PAI form completely, accurately and timely. Maintain a high level of professional expertise and serve as a resource to the rehabilitation unit and the hospital.

Position Insight: 

Set prescribed IRF-PAI completion dates for the multidisciplinary team.Analyze FIM scoring to make certain that FIM scoring is accurate; question appropriately any FIM discrepanciesReview each IRF-PAI in total for accuracy before inputting and finalizingCoordinate IRF-PAI processes with various hospital departments to ensure timely billing and that there is appropriate retention of the IRF-PAIEnsure that the IRF-PAI is entered into the appropriate medical record in cooperation with medical records personnelEnsure that the IRF-PAI is entered into the appropriate medical record in cooperation with medical records personnelAssist in the assessment of patients referred for Acute Inpatient Rehabilitation services by gathering pertinent information, communicating with the Medical Director; initiating the admission process when appropriate and communication with all interested partiesProvide instruction/training on FIM Scoring for Rehab staff and other hospital employees when necessary.Establishes and maintains a therapeutic relationship with patients/significant others.Reports risk management and patient safety concerns to appropriate person according to procedures. Participates in Performance Improvement initiatives. 

Applicants Job Requirements Include:

Education:

Required: Graduate of an accredited Nursing Program.Preferred: Bachelor’s Degree.

Experience: 

Preferred: Experience with PPS and the IRF-PAI processes.Required: Excellent communication skills; detail oriented; able to meet strict timeframes; computer skills; able to work in a team; critical thinking abilities; able to work independently; strong organizational skill.Required: Able to not only pass the FIM competency test, but become an expert in the FIM system.

Licenses/Certificates: 

Required: Current Texas Registered Nurse Licensure required.Required: Current BLS certification.
