Assists with the accuracy and completeness of medication history collection across the continuum of care beginning in the emergency dept or the inpatient units and under the direct supervision of the pharmacy departmentWorks with the patient, family, or caregivers to establish the most accurate medication history of the patientProvides support to the physician and pharmacy staff when clarification of home medications are neededCompares electronic medication records to the patient provided information to ensure accuracyRequirements:Education: HS Diploma or GED equivalentLicensure: Licensed by the Indiana State Board of Pharmacy Tech as a Pharmacy Technician.Experience: Four (4) years experience as a pharmacy tech required. Ambulatory or retail experience preferred. Excellent written and verbal communication skills with people from diverse backgrounds and at all levels within the medical center and in the community. Ability to create and foster a cooperative teamwork environment. Basic computer skills, electronic data entry. Knowledge of commonly used prescription drugs including brand/generic names, available dosage, frequencies and indicationsOur system includes:
