Position Summary Full-Time All Shifts Available Shift

Performs specialty functions in support of medication reconciliation. Obtains a current medication list for admitted patients through in-person interviews. Performs second sourcing by collecting medication list and verifying this with a second source.

Obtains the patient’s most accurate and current home medication list, including prescription and non-prescription medications. Obtains the patient’s medication allergy history. Resolves home medication list discrepancies with an in-patient hospital pharmacist and/or an MMC approved second source of information. Accurately documents the home medication list and allergy information in the Cerner Health Information System. Performs all other duties as assigned.

Required Experience

Education:

High School Graduate or equivalent required.

Experience:

 Minimum of six months experience in healthcare preferred.

Other Knowledge/Skills/Abilities:

Demonstrates excellent verbal communication skills. Demonstrates excellent interpersonal skills. Ability to input data accurately and efficiently into the Cerner Health Information System.
