SUMMARY 
Rochester Regional Health (RRH) is seeking a highly qualified and detail-oriented Medication Safety Officer to join our comprehensive and diverse integrated health system. As a Medication Safety Officer, you will play a crucial role in developing, implementing, and overseeing medication safety programs across our expansive network, encompassing acute care facilities, ambulatory care centers, home care services, hospice programs, and serving both rural and urban areas.

STATUS: Full Time, 40 hours      
LOCATION: All RRH Finger Lakes sites
DEPARTMENT: Pharmacy
SCHEDULE: Monday-Friday, day shift. Hybrid model if desired.  

ATTRIBUTES

BS Pharmacy or PharmD required

Active NYS Pharmacy Licensure required

Completion of American Society of Health Systems Pharmacy (ASHP) accredited residency program, or 3 years of experience in clinical practice preferred

Relevant experience in medication safety, quality improvement, or a similar role within a large integrated health system

Strong understanding of regulatory requirements related to medication safety, adapting to the complexities of diverse care settings

Excellent communication and interpersonal skills to effectively collaborate with a wide range of healthcare professionals across various locations

Analytical and problem-solving abilities to identify and address medication safety issues in different care environments

Certification in Medication Safety or Patient Safety is preferred

Human Factors trained preferred

Just Culture champion certification preferred

RESPONSIBILITIES 

Develop and maintain medication safety policies and procedures tailored to the unique needs of diverse care settings within RRH

Collaborate with healthcare professionals across various departments to identify and address medication safety concerns specific to each care setting

Investigate and analyze medication errors, adverse drug events, and near misses across the entire health system to implement corrective actions and prevent future occurrences

Provide education and training to team members on medication safety best practices, adapting strategies to each care environment

Stay informed about the latest developments in medication safety, pharmacovigilance, and regulatory requirements, ensuring system-wide compliance

Act as a liaison between departments and locations to facilitate communication and promote a consistent culture of safety throughout RRH

Participate in system-wide quality improvement initiatives, contributing to the development of strategies that enhance patient outcomes across RRH

Apply knowledge of safety science, care delivery processes, and pharmacology to design safe systems of medication use

EDUCATION:

BS: Pharmacy (Required)

LICENSES / CERTIFICATIONS: 

LPharm - Licensed Pharmacist - New York State Education Department (NYSED)

PHYSICAL REQUIREMENTS:

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

$126,400.00 - $167,500.00

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
