Responsibilities Job Description Job Summary: The Medication Scribe is responsible for gathering and documenting patient medication information into the electronic health records system. They also assist with related administrative tasks to enhance efficiency and productivity of the care team and improve patient satisfaction. Job Expectations: Prepare current patient-specific medication lists following department procedures, using information from patients, families, pharmacies, and other care facility records. Identify and clarify obvious drug-related issues in the medication regimen. Efficiently triage patients to ensure an accurate medication list is available for physician review, utilizing communication methods like email, fax, or direct contact with patients or providers. Enter medications and allergies into patient profiles as required by protocols. Perform other administrative duties as assigned. Qualifications Required: Experience: 1-2 years of healthcare experience and/or equivalent educational experience. Preferred: Education: Vocational/Technical Training or Associate Degree. Experience: 2 years of experience as an LPN, CMA, CNA, TMA, or in a pharmacy environment. At least 6 months of experience conducting medication histories or similar patient-focused communication. License/Certification/Registration: LPN, CMA, CNA, TMA, or Certified Pharmacy Technician. Additional Requirements: If hired without 2 years of healthcare experience or medication history education/training, completion of a PTCB-Recognized Medication History Education/Training Program may be required within 6 months of hire. These qualifications highlight the preference for candidates with direct healthcare experience or specific training related to medication histories.
