Scope of Responsibilities:  The centralized MTM pharmacist will be responsible for providing remote support to the drugstores by delivering patient care in the form of medication therapy management and various adherence interventions. They will also support the store teams with various clinical programs.

 

Job Summary: Use Outcomes, Equip and other platforms to conduct recommended patient interventions/comprehensive medication reviews. Responsible for conducting payer-driven patient interventions such as CMRs, TIPs, and other clinical interventions that have a positive impact on DIR fees and drive adherence, improved patient outcomes, and pay for performance results. Responsible for reaching out to patients on behalf of their local Kinney pharmacy and providing recommended medication consultations. Responsible for reaching out to providers and suggesting therapeutic changes as needed. Responsible for communication within stores to maximize clinical interventions.

Job Duties:

Review records of patients to determine appropriateness of medication therapy.Evaluate patient’s condition to ensure all issues are being treated.Identify untreated health problems and refer patients to appropriate care.Develop effective medication plans that minimize risk of adverse events.Consult on dosages, administration, CI’s and ADRs.Assess the effectiveness of drug therapy.Collaborate with healthcare professionals to ensure optimal patient care.Keep accurate documentation of medication plans and patient progress.Adherence Improvement- Provide adherence counseling to non-adherent patients.  Offer adherence improvement programs such as ReadyScripts and extended day supplies.Work extensively in Microsoft Excel to utilize various reporting tools- run and merge reports on a daily basis.Helping centrally with the rollout of various clinical programs such as smoking cessation, oral contraceptives, long acting injectables, test to treat and vaccinationsMust be willing to adapt to changes in terms of daily tasks and workload balance.Responsible for keeping current with changes and trends in our industry including regulatory and government affairs.Responsible for completing all mandatory and regulatory training programs.Perform other duties as assigned Qualifications

Educational Requirements:

Pharmacy Degree from an accredited College of PharmacyPharmD Degree preferredLicensed Pharmacist in New York State or Vermont- must have licensure in both states within 120 days of hire.

Experience Preferred:

Preferred:   3-5 Years of practice in a traditional retail setting

Special Conditions of Employment:

Any and all additional eligibility requirements based on the specific positionDrug TestInitial and continuous exclusion and sanction/disciplinary monitoring

Compensation

 

$40.00 per hour

 

The final offer will be determined after careful consideration of multiple factors such as relevant skills, years of experience, and education.
