Job Description:

We are seeking a CPhT to assess medication history for patients in the ER!

Responsibilities:Conduct medication history assessments for new admissions.Document allergy history and medication compliance.Prompt patients for additional medications, including OTC and herbal products.Enter preferred outpatient pharmacies into the electronic health record.Update medication histories in the electronic health record promptly.Ensure accuracy and follow-up on incomplete information.Communicate effectively with patients and healthcare professionals.Collaborate with nurses, pharmacists, and prescribers.Navigate hospital tracking boards for patient selection.Prioritize tasks in a high-volume environment.Work independently and assist coworkers as needed.Report medication history errors to a supervising pharmacist.Complete reports and participate in performance improvement activities.Contribute to the Pharmacy Department's safe and efficient operation.Adhere to policies, procedures, and safety standards.Complete annual education assignments and participate in quality improvement initiatives.Perform other duties as assigned.Requirements:Certification as a pharmacy technicianRetail pharmacy 3-5+ years of experienceShift / ScheduleYou will train on day shift - 6:00am or 7:00am - 2:00pm or 3:00pm for the first 240 hours (6 weeks) -- could flip to their schedule earlier if trained sooner!!After your training period you will switch to 2nd shift - 2:30pm -11:00pm (some flexibility) / primary week days -- every 3rd weekend 8:00am - 5:00pm, BUT you will be off 2 days the following week!Pay and Benefits

The pay range for this position is $19.00 - $22.00
