Memory Care Director

*Starting salary is $70000 per year!

The Memory Care Director (Director, Artis Way Experience) will ensure the culture of the company is cultivated and supported throughout the community by practicing the “Positive Partnerships” philosophy and all programs associated with it. The DAWE will provide comprehensive initial and on-going assessments of resident’s social, emotional, and general well-being, and document appropriately. Working closely with the Director of Health and Wellness, the DAWE will provide assistance with the development of Care plans, ensuring each plan fully captures the unique and individual needs of each resident.

The Memory Care Director will:

Promote The Artis Way philosophy in all departments and work closely with department leaders to support the daily practice of the program. Schedule, coordinate, and facilitate Team Member Orientation for all new hires. Manage the Positive Partner recognition program. Promote awareness and participation in the nomination process. Oversee the selection process and ensure the monthly finalist receives personalized recognition. Complete and submit the Positive Partner of the Year applications. Prepare and manage the annual budget for Positive Partnership programs (i.e., recognition programs, training, monthly Positive Partner program). Partner with the Director of Sales and Marketing to assist in the development of relationships with referral sources. Participate in sales presentations, speaking engagements, and sales calls. Partner with the Director of Business Services to review utilization of programs such as EAP, VoiceFriend, and Positive Partner of the Month program. Develop a strategy to increase and support participation and engagement as needed. Partner with the Director of Life Enrichment to review the program offerings and calendar on a monthly basis. Partner with the Director of Culinary Services to review menus, times of meals provided, and other nutritional needs of residents to ensure personalization. Participate in the recruiting and interviewing process. Introduce The Artis Way to candidates to ensure their ability to adopt the philosophy and successfully assimilate to the Artis culture.

Case Management:

Conduct admission procedure of new residents, (including completing all forms, entering all admission data into computer, and documenting their social and ADL needs. Comprehensively assess and document resident’s social, emotional, and general well-being, and document appropriately for initial case management. Update regularly and as events warrant. Assist the Director of Health and Wellness with the development of Care plans, ensuring that each plan fully captures the unique and individual needs of each resident. Collaborate with department leaders and families to coordinate move-in activities and ensure a smooth transition for each resident. Assist residents with their transition to community life. Provide introductions to team members and residents, discuss meal service, and engage residents in enrichment activities. Complete annual case management as mandated by, and in accordance with, state rules and regulations. Coordinate and facilitate family care plan meetings, in accordance with state regulations and Artis standards. Act as the moderator of meetings, document topics discussed, and perform all follow up as appropriate in a timely manner. Communicate all outcomes with families.  Maintain networks with external organizations that specialize in memory impairment issues to stay up to date on current trends in research, treatment, and evidence-based best practices. Attend continuing education opportunities through webinars, local and state conferences, and CEU programs. Maintain effective communications with residents, families, and providers when out on MLOA. Assist facility with discharge planning as needed. Attend care plan meetings when resident is in rehab and/or SNF to assist with transition back to Artis. Observe community policies and procedures regarding resident and team member safety, attendance, timeliness, house rules, teamwork, and resident service. Maintain professionalism and resident confidentiality at all times. Adhere to all Infection Prevention, OSHA, fire, and safety regulations at all times, to ensure the safety and well-being of all community residents. Perform all other duties as requested.

Educational and Licensure Requirements:

Bachelor’s degree or higher in Nursing, Social Work, Psychology, or Gerontology preferred. 3 or more years of experience working in senior living or healthcare environments.
