Southview Manor (formerly Southview Care) is an inner-city skilled nursing facility (SNF) with 5 floors and an average census of 160+. We have and multiple levels of care, including long term care and a large behavioral health (formerly indigent) population.
Highly ranked Quality Measures – this means we provide excellent clinical care!The average age of our patients is 35-55 and the most common diagnoses are schizophrenia, bi-polar and major depressive disorders.Many are able to age in place, and about 25% return to the community.We have several programs encouraging engagement, but the most popular spot in our center is the game room. Job DescriptionWhat you will do in this role: The Psychiatric Rehabilitation Services Coordinator (PRSC) provides group and individual therapy. This includes psychiatric rehab and case management services to adults with a history of psychiatric (Behavioral Health) hospitalizations. You will help them with long-term care and/or stabilization. You will be a part of our interdisciplinary team in developing and implementing therapeutic services and interventions to best serve their needs. Our patients are interactive, motivated and eager to go on this journey with you.

What we need from you:
Prior experience as a PRSC or mental health professional in a licensed nursing home, intermediate care facility, or specialized mental health rehab facility (SMHRF) is preferredBachelor’s Degree in a human services field (including but not limited to sociology, special education, rehabilitation counseling or psychology) and have a minimum of 1 year of supervised experience in mental health or human services settingExperience with Point Click Care (PCC) is ideal, but not requiredIf you are looking for an opportunity that provides satisfaction and personal growth, we encourage you to apply.
