This position will be working 24 hours/week during weekdays on day shift. Traveling to see patients in the Portland area is required.
This position provides specialized case management mental health services to participants under the PACE model of care. Spends a majority of time in direct patient care in clinic and home-based environments. Working with other team members to provide comprehensive assessments and consultation.

Required Qualifications:
Master's Degree in Social Work from a CSWE Accredited School of Social Work, OR from a school in candidacy status by the council of Social Work Education (CSWE). Oregon Clinical Social Worker License upon hire. Driving is necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. 2 years of relevant Social Work experience. May include CSWA experience.Preferred Qualifications:
