Job Description

JOB SUMMARY:

Performs emergency and scheduled psychiatric evaluations, individual therapy, group therapy and case-management functions to clients in mental health programs associated with Shore Health System.

ESSENTIAL FUNCTIONS OF THE JOB:

1.  Conducts intake interviews, psychosocial and mental status evaluations for primary psychiatric triage and care.

2.  Develops an individualized plan of care for each client in collaboration with the multidisciplinary treatment team.

3.  Provides individual therapy, group therapy and case management functions.

4.  Monitors clients' responses to treatment and revises the plan of care.

5.  Collaborates with the Psychiatrist, Director of Behavioral Health Services, referral sources and community agencies.

6.  Maintains patient confidentiality.

7.  Maintains client records in accordance with Behavioral Health Services policies.

8.  Identifies opportunities for improvement, promotion and provision of services, including marketing.

9.  Maintains positive customer and staff relations.

10.Identifies own educational needs for professional growth and participates in the development of annual goals.

11.Strives to achieve excellence of service as described in the CARE Manual.

12. Responds to the following Shore Health System codes:  Code Green, Code Red (in the immediate area), Code Pink (when called), Code D (when called).

13.Responsible for honest behavior in all matters.  To the best of the employee’s knowledge and understanding, complies with all Federal and State laws and regulations.

14.Maintains the privacy and security of all confidential and protected health information.  Uses and discloses only that information which is necessary to perform the function of the job.

15.Performs related duties as assigned.

Qualifications

Master's degree from an accredited school of nursing, social work, or counseling required.  LCSW-C  (Licensed Clinical Social Worker - Certified) OR LCPC (Licensed Professional Counselor, OR RN with APRN (Clinical Nurse Specialist) required.

Candidates with LGSW or LGPC will be considered,but must receive LCSW-C or LCPC within 1 year.

Minimum of two years of outpatient psychiatric nursing or social work experience in a mental health setting, preferred.

The employee in this position must be able to demonstrate the knowledge and skills necessary to provide care based on the age of the patients served.  He/she must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status, to interpret the appropriate information needed to identify each patient's requirements relative to his/here age-specific needs, and to provide the necessary care as described in the department's policies and procedures.
