The Medical Social Worker collaborates with themultidisciplinary team to provide input in the development of the plan of carefor those patients requiring social work intervention. The Medical Social Worker provides medicallynecessary intervention to patients who have social or emotional problems thatare, or are expected to be, impediment(s) to the effective treatment of themedical condition and/or rate of recovery. Additionally, the Medical Social Worker provides these services inaccordance with physician’s orders.
Essential Duties and Responsibilities:
+ Ensures clinical documentation is completed timely throughout the course of treatment.
+ Establish therapeutic rapport with patients and families/loved ones.
+ Carries a case load of 10-15 patients in OP.
+ Facilitates admission assessments and the admission process for new patients with professionalism, courtesy, and a welcoming attitude.
+ Understands role in providing expeditious access to care for individuals seeking services.
+ Participates in weekly treatment team and communicates weekly treatment goals with all team members.
+ Completes appropriate number of therapy sessions weekly for patients, including family/support system sessions aligned with the patients’ level of care.
+ Facilitates process orientated and psychoeducation groups one to two times, daily in OP.
+ Effectively manages crises in sessions and the milieu.
+ Works collaboratively with the entire treatment team and milieu staff to always ensure the highest level of clinical quality.
Minimum Requirements:
+ Baccalaureate degree in Social Work (BSW) or Master’s degreefrom a school of social work accredited by the Council on Social Work Educationper state/client requirements required
+ Minimum of one (1) year of social work experience in ahealth care environment in the last two years preferred
+ Complies with all relevant professional standards ofpractice
+ Participation and completion of Amergis' Competency programwhen applicable
+ Current CPR if applicable
+ TB questionnaire, PPD or chest x-ray if applicable
+ Current Health certificate (per contract or stateregulation)
+ Must meet all federal, state and local requirements
+ Successful completion of new hire training as applicable tojob site
+ Understand patient confidentiality and HIPAArequirements
+ Ability to effectively elicit/provide information to andfrom appropriate individuals (including, but not limited to, supervisors,co-workers, clients) via strong communication skills; proficiency in theEnglish language is required
+ Computer proficiency required
+ Must be at least 18 years of age
