As the MHRS Facilitator your responsibilities will include managing case coordination, documenting mental health notes and care plans, and ensuring timely communication across all supporting parties involved. You will work closely with a diverse team of professionals to provide comprehensive care and support to children and their families.

 

ABOUT MARIN WRAPAROUND

The Marin Wraparound program supports Marin County children and adolescents in the Child Welfare, Probation, and Mental Health systems who are transitioning from or at risk of entering out-of-home care or juvenile hall. The program serves youth aged 6-18 with the primary goal at keeping youth in family-like settings. Team members work with children and families providing services almost exclusively in the community: in families’ homes, schools, other community locations.

Responsibilities Facilitate Child and Family Planning Meetings, including preparation, coordination, facilitation, note-taking, and follow-upManage cases and coordinate care with county and community partners (e.g., social workers, therapists, probation officers, teachers)Complete all necessary documentation, including mental health notes, Family Team Meeting notes, and incident reports in a timely and accurate mannerProvide crisis management and intervention, including behavior management if necessaryAttend team meetings, trainings, and supervision sessionsFollow clinical best practices and complete required clinical documentationParticipate in emergency on-call response once every quarterUtilize crisis communication and de-escalation techniques as needed Qualifications

REQUIRED

Bachelor’s degree in Counseling/Psychology/Social WorkMust be eligible for MHRS certification (4 years of experience  providing services in a mental health setting)Flexible schedule with the ability to work some evenings and weekend hours as neededBe part of a rotating emergency on-call system, scheduled ahead of timeMust be at least 21 years of ageValid driver's license, clean driving record, and insurability through Seneca Family of Agencies’ insurance policyTB test clearance, fingerprinting clearance, and any other state/federal licensing or certification requirementsProof of up-to-date COVID-19 vaccination
