Our social workers support an integrated and comprehensive Performance Improvement program for MacNeal Health Network. Objectively and systematically assess, monitor, evaluate and improve the processes that affect the quality of patient care, ensuring compliance with: MacNeal Health Network mission and values, The Care Management Program, rules and regulations, MacNeal policies and regulatory standards.
Utilizing established practice and professional standards, this position assesses and provides intervention concerning disability resulting from emotional, social and economic stresses of illness; identifies and provides interventions to meet the psychosocial needs of patients and their families; resource for complex discharge planning needs.

Requirements:
A Master’s Degree in Social Work. Minimum of six (6) months of prior social work experience in a hospital setting. Current certification by the State of Illinois as a Licensed Social Worker. (L.S.W.).
