Position Summary:

The MSW Case Manager is a professional who collaborates with physicians, health care team members and patient/families to coordinate care for specific patient populations across the continuum and between care settings during an episode of illness. Provides various social work services to meet needs of patients and family members. Services include psychosocial evaluation of patient and relationship to family members in considering patient medical needs and prognosis.

What the Case Manager will Need:

Masters degree in social work required.State of Michigan, social work license required (LLMSW, LMSW)Demonstrate ability and skill in the management of patient care planning and continuous quality improvement in services deliveredRecognized ability to be a key contributor as a member of a patient care team

What the Case Manager will do:

Performs social work services for patients found to be at risk including, but not limited to the diagnosis of substance abuse, psychiatric problems, domestic violence, pediatrics, end stage dementia, developmental disabilities and CVI. Completes psychosocial evaluation of patients and coordinate transition through the continuum of care.Develops social work treatment plan and provides individual and/or family counseling as appropriate. Evaluates the socio-emotional status of patients and family members as necessary.Provides various direct services including: crisis intervention, counseling, discharge planning, coordination of services post-hospital care, refers for financial assistance, makes referrals for community services as necessary to assist the patient in transitioning along the health care continuum.Is a resource to patient, family and staff (and other health care professionals) by providing education specific to the emotional impact of medical treatment on patients and family members. Instructional sessions to include other related topics
