The Specialist will coordinate, organize and develop community outreach and partnerships throughout Onondaga County. They train and monitor outreach activity being done by the other Navigators and assist individuals and families with completing the application process who are potentially eligible for Medicaid, Child Health Plus or other available insurance plans offered on the New York State of Health Marketplace in accordance with the Affordable Care Act.
Responsibilities Become an expert on the Affordable Care Act. Coordinate and liaison with outside agencies and companies to hold Outreach events.Work with community partners to develop referral contacts and develop relationships with referral agencies, including the TSA Adult Rehabilitation Center, Medicaid, SYTE, Onondaga Nation, Teen Challenge, Probation, and others as assigned. Represent the program at outreach events as requested at various sites in Onondaga County, including health fairs, school open houses, NYSOH and other community events.Develop an outreach strategy each year and present it to the Navigator team. Maintain records of outreach events including contacts, attendance, types and amounts of materials, etc. Complete GIK forms for events as needed.Become an expert in the small business exchange and Navigate and assist small businesses in the small business exchange. Complete and maintain the Site Schedule, submit to NYSOH monthly.Assist with outreach training for the Navigators.Attend the New York State of Health Conference Call bi-weekly.Navigate the NY State of Health to help enroll individuals and families in health insurance.Assist approximately 40 households per month with completing all applications; assist clients with obtaining all supporting documentation; document contacts with prospective enrollees; follow-up with completed applications as necessary.Maintain database and client records as appropriate.Attend all meetings and trainings required by The Salvation Army and the NY State of Health. Qualifications Associates degree preferred. At least One-year of human services or health insurance experience or equivalent in training and experience.Some flexibility in schedule is expected according to individual client needs and outreach opportunities. This may include early morning, evening or weekend availability to meet with potential enrollees or attend outreach events.
