Overview

As our Navigator RN at CHI St. Alexius Health Dickinson, you work collaboratively with physicians, staff and other health care professionals to provide a medical home and care coordination across the health care continuum for all patients within the physician office setting and is an integral member of the health care team who works to ensure safety, best practice and high quality standards of care are maintained. Is held accountable for policies and procedures of the department and organization.  This position supports the mission, vision, and philosophy of CommonSpirit Health.

Responsibilities
Provide oversight of the disease registry database by assuring database is kept up to date; identify patients overdue for visits, labs, or referrals and arranging for follow-up services as appropriate; identify patients not meeting clinical goals, such as BP control or glucose control, and arranging for follow-up services by protocol or as appropriate; create patient, physician, and clinic level quality performance reports.Provide oversight or conducts pre-visit chart review of patients by identification of all needed preventive health maintenance, immunizations, and chronic disease interventions; when standing orders allow it the interventions may be ordered or completed before the patient sees a provider; fills out pre-visit forms or initiates office visits forms to communicate the review to the provider.Works with patients and families on Self-Management Support by setting short and long-term goals for self-management of chronic disease; addressing medication adherence in patients not meeting outcome goals; works with patient to create a plan for Health Behavior Change utilizing the 5A’s approach;Participate actively in the continued development of improved care practices for patients, including practice guidelines and protocols through the use of evidence-based practice.Works collaboratively with provider to provide exceptional careRecords all care information concisely, accurately and completely, in a timely manner, in the appropriate format and on the appropriate formsMaintains order, stock, and cleanliness of all areas in the Emergency DepartmentUtilizes resources appropriately and follows facility guidelines, policies, and procedures
Qualifications

Required Education 

Bachelor’s Degree in Nursing (BSN).

Required Licensure and Certifications

Active, unencumbered Registered Nurse (RN) license from North Dakota or compact license.American Heart Association Basic Life Support (BLS).

Required Minimum Experience

Minimum of four (4) years of experience as a RN
