POSITION PURPOSE AND SUMMARY
Under the direction of the Neuroscience Transfer Service Manager, the Neuroscience Transfer Service Nurse will oversee the neuroscience transfers coming into the facility for the Comprehensive Stroke Program or for a higher level of care of neuroscience services in accordance with the standards for the disease specific guidelines set forth by the Joint Commission and the state and federal regulations for safe patient transfer. The coordinator will be responsible for answering the transfer line, contacting the physician on service and dispatching the ambulance for transport. In addition, they will work with the Neuroscience nursing personnel to place the patient, activate emergency response teams, and create the admission for the incoming patient. The nurse will manage transfer logs, create patient feedback reports and provide relevant information regarding transfer to the Neuroscience leadership and physicians. The Neuroscience Transfer Service Nurse will work with leadership on performance improvement as necessary. They will also be responsible for guiding patients and families through the transfer process, developing relationships with service providers and tracking interventions and outcomes. The nurse will support research clinical trials and programmatic initiatives. They will work closely with nursing supervisors, nursing directors, nurse managers, case managers, and administration for making decisions related to bed planning and bed assignment. The Neuroscience Transfer Service Nurse will perform all duties in a manner which promotes a team approach to patient throughput and reflects the mission and philosophy of Powers Health.
EDUCATION/EXPERIENCE:
Licensed registered nurse with two years of direct patient care experience in an acute care hospital Valid Indiana RN license required 2 years of relevant experience in critical care, emergency department, or ICU preferred Must demonstrate excellent communication and problem-solving skills along with the ability to multi-task. Must be flexible in work hours. Computer knowledge and PC keyboarding skills required.
