Our Case Manager provides a range of case management including social casework, engagement, mentoring, and community referrals to service providers, as applicable that extends beyond the initial resettlement period, specifically focusing on medical, educational, housing, and clients’ special needs as well as conducting a mental health screening for all clients, making timely, appropriate referrals to mental health care providers. Assumes primary responsibility for all persons served on assigned caseload, including intake, assessment, periodic reviews, and treatment planning. Responsible for all reports, progress notes, collateral contacts and assignments as required. Assume responsibility for all case files on assigned caseloads.Provide a range of case management, including social casework, engagement, mentoring, and community referrals to service providers, as applicable that extends beyond the initial resettlement period, specifically focusing on medical, educational, housing, and clients’ special needs.Conduct a mental health screening for all clients and make timely, appropriate referrals to mental health care providers.Provide information, referrals, and outreach services regarding community resources, including creating and distributing of the resource guide.Participate with Clinical Manager and other staff in the care planning and goal setting for the growth and development of vulnerable individuals served. Coordinate the development, implementation, and evaluation of Person-Centered Planning. Work collaboratively with referral sources, agencies, institutions, and individuals who have important roles in the client’s life situation. Participates in in-service trainings and conferences related to the program.Conforms with and abides by all regulations, policies, work procedures and instructions. _________________________________________________________________________________Job Qualifications
Education, Training, and Licensure/Certification
Master’s degree in human service-related field, or equivalent experience working with refugees or other vulnerable populations.Minimum one year experience in case management in a culturally diverse environment, refugee related casework or community services preferredKnowledge Skills and Abilities
Ability to work effectively in a cross-cultural context. Ability and willingness to work with refugees compassionately, professionally, and effectively. Understanding or willingness to quickly learn the characteristics, behaviors and needs of the refugee populations.Excellent organization and prioritization skills; ability to utilize computer technology and various applications, including spreadsheets.Ability to professionally and effectively communicate orally and in writing. Ability to facilitate and collaboratively work with the treatment team to establish and implement treatment plans and goals. Ability to develop outcomes measurements related to Person Centered Planning goals.Additional Work Requirements
Unrestricted, valid driver’s license with good driving record. Reliable personal transportation suitable for safely transporting persons served, proof of current automobile insurance.Physical and Mental Requirements
Stands and moves about indoors and outdoors.
