New Life Recovery Center is searching for a diverse, energetic, self-starting Case Manager to join our talented and growing team. 

Case Managers provide case management for individuals with mental illness, substance use, or co-occurring disorders who demonstrate a need for active case management services. The case manager coordinates services and/or resources needed by the client to support their basic needs such as psychiatric/MAT appointments, medical appointments, and court hearings. Also provides for follow through and continuity of care including discharge, referral, placement, follow-up appointments and liaison services. Attends weekly treatment team meetings and maintains contact with referral sources (e.g., Probation, DSS, etc.) as needed.

 

Starting Hourly Rates:

Case Manager I: $16.73

Case Manager II: 17.56

 

Education Requirements:

Case Manager I:

High School Diploma is required. Bachelor's Degree in Education, Social Work, Psychology or Human Services related field is preferred.

Case Manager II:

Bachelor's Degree in Education, Social Work, Psychology or Human Services related field is required. Registration as a QMHP or trainee is required. Must maintain QMHP status and all required trainings/CEUs throughout employment. CSAC preferred.

Additional compensation will be considered for any new employees who speak multiple languages
