Position Requirements:
Graduate of an Accreditation Council on Pharmaceutical Education (ACPE) accredited school or universityDoctor of Pharmacy preferredAny State License1 year of recent Inpatient experience requiredBLS
Position Responsibilities:
Assist patient's response to drug therapy and planning drug therapy based on physician established diagnosesOrder and evaluate laboratory test necessary to evaluate drug therapy effects and outcomesMonitor and manage pharmacotherapy requiring periodic adjustment due to specific or changing pharmacokinetic characteristicsEvaluate medical and medication histories for drug-related problems and adjusting drug therapy accordinglyConsult with other healthcare providers about patient treatment needs or optionsCompound medications as requiredReview patient profiles at the time of dispensing in order to closely monitor medication therapy
