GRADE: C7 NOTICE OF POSITION OPENING POSITION: Non-Categorical Case Management & Community Supports PROGRAMS: Adult Mental Health DIVISON: Center for Counseling & Psychological Services SUPERVISED BY:  Case Management Team Lead SUMMARY OF POSITION:  Seeking a collaborative, energetic, team-oriented, creative individual to provide a complement of services to meet the support needs of adults in mental health services.  This is a full-time benefited position primarily based in the community. ESSENTIAL FUNCTIONS: + Provide support services to clients that include but are not limited to outreach visits, support services, accessing benefits, assistance with independent living skills and transportation of clients in personal vehicle when clinically indicated. + Develop and/or deliver services to assist clients in gaining access to needed medical, social, educational, vocational, housing, financial and other services. + Assist clients in developing fulfilling lives by collaborating with them in implementing their treatment goals in the community. + Assist in developing natural support networks for adults in accordance with their treatment goals. + Collaborate and consult with clients, case managers, clinicians, community partners, hospitals and others involved with the treatment team on an on-going basis.  Actively participate in treatment team meetings. + Attend required meetings and trainings, including individual and/or group supervision and other agency committee work. + Work collaboratively with other agencies providing services to clients in accordance with treatment goals. + Attend staff meetings at WCMH as required. + Other duties as assigned by supervisor. SUCCESSFUL CANDIDATES WILL HAVE THE FOLLOWING SKILLS: + Therapeutic experience with target population. + Understand trauma informed services and have experience with trauma survivors. + Working knowledge of family systems. + Excellent case management skills. + Reliable, on-time, flexible and be able to work as part of a team and independently. + Must be able to complete all physical requirements of the job with or without reasonable accommodation: + Remain in a stationary position up to 50% of work hours. + Able to alternate between sitting and standing as needed. + Drive to different client homes using own vehicle. + Ability to lift and carry up to 50 pounds to assist a client, if needed. + Ability to enter text or data into a computer by means of a traditional keyboard. PHYSICAL REQUIREMENTS:  The role may involve the following physical requirements: + Prolonged periods of sitting and standing during therapy sessions and administrative tasks. + Effective verbal communication, including the ability to clearly convey information both in-person and over the phone. + Basic manual dexterity to handle documentation and computer-related tasks. + Mobility to move within the office environment and between different work locations.
