Job Overview:
The Patient Navigator, reporting to Principal Investigator and Project Director, will monitor Hepatitis B and Hepatitis C screening in the ED, inpatient, and outpatient locations in addition to working with the project implementation team to expand screening programs to other hospital locations and include HIV screening to the program. Through daily EMR reports the Patient Navigator will work to identify positive patients, contact those patients with results, provide education, counsel on the importance of follow up care, and navigate linkage to care with a HBV, HCV, or HIV provider and with primary care if absent. Care management will also include follow-up to ensure engagement in care, provide patient support utilizing a referral network of primary care, specialty care, social services while developing relationships with federally qualified health centers and other community-based organizations, thus establishing a provider network consistent with referral protocols based on geography and insurance coverage. Patient Navigators will assist uninsured patients with insurance enrollment.
The Patient Navigator will support the program with clinical staff education which includes identifying eligible patients, the importance of Hepatitis and HIV testing, and general support to the logistics of the screening program. The Patient Navigator will work with physicians and nurses to provide support in answering patient questions and provide counseling as needed. In addition, the Patient Navigator will collaborate in maintaining a database of positive patients and patient s compliance with first linkage to care appointment. Patient Navigator will be involved with additional tasks including community outreach and education events, developing patient education materials, and disseminating work including preparing presentations, abstracts and posters. PN will also assist as needed with other functions at the Florham Park Multispecialty practice.

Required:
Bachelor s Degree in a Related FieldMinimum of two years of professional experience in a healthcare/public health, which includes responsibilities for patient education and linkage to care Prior experience in infectious diseases preferredScheduling Requirements:
