Change Grow Live is seeking an experienced NMP to join our Kirklees service on a full time or part time basis.
In this role, you will play a vital part in delivering high-quality, person-centered care to people on our opiate cohort, some of whom face complex physical and mental health challenges alongside substance use. Working as part of a multidisciplinary team, you will provide clinical assessments, prescribe appropriate treatments, and support individuals on their recovery journey.

Full Time Salary Range: £47,792.23 - £53,329.99 dependent on experience (based on full time hours, pro rata for part time)

ResponsibilitiesChange, Grow, Live are the lead provider of the Integrated Drug and Alcohol Treatment Services in Kirklees.
The successful applicant will join an established and dynamic service with a large clinical team and aims to help people improve their lives. We are already a high performing service, with a supportive culture and a real focus on supporting people to achieve their goals.
The role will include working flexibly maintaining the most appropriate level of service provision. This will include at least one evening per week until 8pm (Tue/Wed/Thu) with an early finish on another day structured in your diary, as well as weekly CPD. The role is based on site daily and is not suitable for hybrid working.
We would however consider applicants who wish to work agreed part time hours from 0.5FTE to full time.
We are interested to hear from qualified NMP applicants who have experience of prescribing in a community service and are either a first level registered nurse with a current PIN and three years post qualification experience or a qualified pharmacist with current GPhC registration and two years’s post-qualification experience.
The successful post holder will need to undertake a DBS check at Enhanced Level (Adult and Child).

Assess, diagnose, plan; implement and evaluate treatment/interventions and care for service usersPrescribe and review medication for therapeutic effectiveness appropriate to service user needs and in accordance with evidence-based practice and national and local protocols, and within scope of practicePrioritise health problems and intervene appropriately to assist the service user in complex, urgent or emergency situations seeking immediate advice and direction from the Clinical Lead if neededAssess, update, and refer service users for BBV vaccination and immunisation programmes. Support service users to adopt health promotion strategies that promote healthy lifestylesCommunicate effectively with service users and carers, anticipating barriers to communication and recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicatingMaintain effective communication within the organisational environment and with external stakeholders, acting as an advocate for service users and colleagues where necessaryProduce accurate, contemporaneous and complete records of consultation, consistent with legislation, policies and proceduresAssess effectiveness of care delivery through self and peer review, benchmarking, and formal evaluation. Be actively involved in the annual appraisal systemFull driving licence and access to own transport, or ability to demonstrate easy movement between services, if necessary
