**OVERVIEW:** AdventHealth Medical Group is seeking a NP to provide comprehensive care, support and education to patients and their families through all aspects of their Pre-Admission process. The NP will be responsible for assessing patient needs and planning, implementing and evaluating their progress, based on knowledge inherent in their practice utilizing a multi-disciplinary approach. This is a 0.6 FTE position (24 hrs/week) working Tuesday, Friday and every other Monday **JOB RESPONSIBILITIES:** + Pre-operative evaluation including histories and physical exams, clinical evaluation and assessment of the peri-operative surgical patient for surgical clearance + Responsible for facilitating communication between offices, surgeons, and the anesthesia team + Ensures that appropriate testing and clearances are obtained prior to surgery and communicates with the Surgical Optimization physician or NP regarding patient concerns + Enters labs and testing as appropriate + Formulates collaborates plan of care and identifies expected patient outcomes + Organizes, coordinates and prioritizes patient care utilizing available resources + Provides patient and family education + Completes a thorough chart review and documents pertinent medical history, medication reconciliation, database assessment, and risk and triage when appropriate + Adapts to rapidly changing technology as evidenced by competency in technology and equipment used in the patient care setting. + Documents patient status, interventions, response and outcomes in accurate, timely manner. **REQUIREMENTS:** + Master’s Degree in Nursing as Nurse Practitioner, from a nationally accredited postgraduate certificate program. Must be a graduate from an Accredited School of Nursing + Associate degree in nursing (Graduate from an Accredited School Of Nursing) + 2 years of experience as a cardiac NP + Registered Nurse Practitioner Licensure for; and listed on the state Advanced Practice Nurse (APN) Registry.
