Job Description:Utilizes advanced health assessment, knowledge, and decision making skills, works independently and in collaboration with physicians and other health care professionals. Provides health care to individuals and families, emphasizing health promotion and disease prevention. They may care for patients ranging in age from newborn to elderly as noted in age-related category and area of expertise.

We are seeking a practitioner (adult-geriatrics trained/certified preferred) to join our Eldercare team in providing exceptional healthcare to our adult patient population; the position will work in our Park Rapids and Detroit Lakes locations. Our collegial Eldercare group is innovative, quality-focused and driven to provide exceptional care for both the patient and their families. Our current team includes 4 advanced practice clinicians. 1.0 FTE, full-time statusClinical hours are Mon-Fri typically between 8a-5pProvide services to seniors in the transitional care units, nursing home/LTC facilities and assisted living campuses in Detroit Lakes and surrounding communities8-10 patients per day in nursing home/assisted living settingsMay be asked to see geriatric patients in the clinic settingCall rotation is 1:5 weekday nights and weekend /holiday call; must be able to respond via telephone within 30 minutes.Must be able to conduct advance care planning with elders & families and provide education to NH staff.Must have the ability to work independently off-site without direct supervision of MD and work well with facility leadership and Medical Director on processes and quality improvement.Prior experience in family medicine, internal medicine, or geriatrics is preferred.

The Nurse Practitioner or Physician Assistant utilizes advanced health assessment, knowledge, and decision-making skills, while working both independently and in collaboration with physicians and other health care professionals. The NP/PA will provide health care to individuals and families, emphasizing health promotion and disease prevention. The primary population served is 60+. Position requires high level of customer service skills to establish and enhance positive relationships with patients, co-workers, and others.


EDUCATION/TRAINING:

Master’s or Doctorate degree in Nurse Practitioner or Bachelor’s/Master’s degree in Physician Assistant program from an accredited program/institution.Adult-Geriatrics trained/certified or willingness to obtain is preferred

LICENSURE/CERTIFICATIONS REQUIRED:

Current licensure as a registered nurse in appropriate states (if NP).Current licensure as a Nurse Practitioner or Physician Assistant in appropriate states.Current certification in CPRNational certification, in applicable area.Successful completion of Essentia Health credentialing process prior to practice.
