TidalHealth in Maryland and Delaware is seeking a compassionate and skilled Hospice and Palliative Care Nurse Practitioner or Physician Assistant to join our interdisciplinary team. The ideal candidate will be committed to providing comprehensive care for patients with serious, chronic, or life-limiting illnesses in both inpatient and outpatient settings. This individual will also have the opportunity to educate residents and fellows, contributing to the development of the next generation of healthcare professionals.
Clinical Care:
Provide expert care for patients with complex symptom management needs, ensuring optimal quality of life. Conduct thorough assessments and develop individualized care plans for hospice and palliative care patients. Facilitate and lead goals-of-care discussions with patients and families, ensuring alignment with the patients’ values and preferences. Complete and document advanced care directives and other essential end-of-life planning documentation. Manage and adjust treatment plans for physical, emotional, and spiritual symptoms, including pain, dyspnea, nausea, and psychological distress.Team Collaboration:
Participate in interdisciplinary team (IDT) meetings to coordinate care and ensure holistic patient management. Communicate effectively with physicians, nurses, social workers, chaplains, and other team members to provide integrated care. Collaborate with inpatient and outpatient teams to facilitate seamless transitions of care.Education and Mentorship:
Educate and mentor medical residents and fellows, fostering their growth in palliative and hospice care principles. Provide ongoing education to team members, patients, and families regarding symptom management and end-of-life care.Conflict Resolution:
Approach patient and family conflicts with empathy and calmness, guiding discussions to resolution while maintaining focus on patient-centered care.
Flexibility:
Be willing to be cross-trained for both inpatient and outpatient palliative care settings. Potentially float to multiple locations, including Nanticoke, as needed.Qualifications:
The NP must be a graduate of an accredited school of nursing or have completed PA schooling. Candidate must be certified and licensed in Maryland at the time of hire. Experience in palliative care, hospice, or related fields preferred. Demonstrated ability to handle complex clinical scenarios and manage patient/family dynamics effectively. Strong communication and interpersonal skills, with a focus on empathetic care. Commitment to ongoing professional development and education.Preferred Attributes:

The approved hiring range is minimum $116,000 and maximum $134,000.
