Works collaboratively with a designated physician to provide a team approach to holistic patient care. The team of MD and RN Oncology Coordinator will navigate their patients through their course(s) of treatment. This approach will provide continuity for the patients and emphasize 1:1 nurse patient relationships along with safe, high quality care. **Requirements:** + RN license – NH or other Compact State + 1 to 3 years + Associate's Degree **Other Skills/Qualifications:** + Strong customer service skills + Strong analytical skills + Prefer oncology experience, minimum of 2 years and/or work in an ambulatory setting; OCN preferred **Major Responsibilities:** 1. Work as a team member with the physician to monitor and manage patient care for their designated patientcaseload. 2. Act as a point person for information and assistance for the schedulers, infusion nursing staff, social work, medical assistants,office staff and other multidisciplinary departments. 3. Meet with patients to review their plan of care, medications, symptoms, and psychosocial issues.Administer injectable therapies as ordered. 4. Perform hospital rounds on inpatients as appropriate. 5. Follow up with all needs designated by the provider. 6. Assist with setting up patient teaching sessions, consults, and concurrent care with other providers. 7. Complete special requests and projects as needed by the physician. 8. Review the charts at the end of each day to ensure that all orders have been addressed. 9. Participate in scheduling rounds, palliative care rounds, clinics, tumor board as appropriate. 10. Be aware of key issues and changes with your team of patients.
