Job Title: Nurse Care Manager
Job Description

Provides telephonic nursing services to patients and families. Collects and analyzes patient data and assists with the development and maintenance of care plans. Ensures compliance with regulatory body standards including Joint Commission and PCMH. The RN roomer will work with the provider performing the duties of a Medical Assistant and patient education. Will perform point-of-care testing and other screenings, expected to be performed in short order every 20 minutes.

The position is based at the Security Square Site, located at 6901 Security Blvd Suite 200, Baltimore, MD. Free parking is available on site. The standard work schedule is Monday to Friday, with shifts from 8:00 am to 4:30 pm or 8:30 am to 5:00 pm, plus one late night per week from 11:30 am to 8:00 pm. No weekends are required. This role offers the opportunity to gain well-rounded experience in Primary Care, working with the uninsured and low-income community, and being exposed to multiple specialties.
