Job Title: Nurse Care ManagerJob Description Provides telephonic nursing services to patients and families. Collects and analyzes patient data and assists with the development and maintenance of care plans. Ensures compliance with regulatory body standards including Joint Commission and PCMH. The RN roomer will work with the provider performing the duties of a Medical Assistant and patient education. Will perform point-of-care testing and other screenings, expected to be performed in short order every 20 minutes. Responsibilities + Provide telephonic nursing services to patients and families. + Collect and analyze patient data. + Assist in the development and maintenance of care plans. + Ensure compliance with regulatory standards including Joint Commission and PCMH. + Work with providers performing duties of a Medical Assistant. + Educate patients on their conditions and care plans. + Perform point-of-care testing and other screenings every 20 minutes. Essential Skills + Registered Nurse (RN) license + Experience in care management + Experience in case management + Ambulatory care experience + Experience in patient education + At least 2 years of experience as an RN in med surg or community health + Experience with infectious diseases and chronic illnesses such as hypertension and diabetes Additional Skills & Qualifications + EMR experience is a plus Work Environment The position is based at the Security Square Site, located at 6901 Security Blvd Suite 200, Baltimore, MD. Free parking is available on site. The standard work schedule is Monday to Friday, with shifts from 8:00 am to 4:30 pm or 8:30 am to 5:00 pm, plus one late night per week from 11:30 am to 8:00 pm. No weekends are required.
