Job Summary:

The Nurse Care Manager works in conjunction with the CPRC team to provide education and support to clients who have or are at-risk for complex medical conditions in addition to mental health and or Substance-use disorders.

Key Responsibilities:

Develop wellness and prevention initiatives.Facilitate health education groups by utilizing advanced knowledge of complex medical conditions.Participate in the initial treatment plan development for all of their Healthcare Home enrollees.Collaboratively develop a treatment plan of healthcare goals for individuals with co-occurring chronic diseases.Consult Community Support Specialists regarding patient identified health conditions.Manage the process of hospital and medical provider admissions and discharges for Healthcare Home patients.Train patients on complex health conditions regarding medical diagnosis, treatment, and medications.Implement, as required, DMH Net Health technology programs and initiatives such as Cyber-Access and metabolic screening.Monitor and report performance measures and outcomes using sophisticated information systems developed for nurses.

 Education and/or Experience Qualifications:

Active RN or LPN licenseExperience in direct patient care preferred Knowledgeable in chronic disease processes in children and adultsExcellent written and verbal communication skills.Ability to implement new innovative systems.Ability to make decisions based on critical thinking and nursing process
