As a Nurse Care Manager, you'll collaborate with internal and external treatment team members to deliver comprehensive care for clients who have or are at risk for complex medical conditions. Your role will involve developing personalized treatment plans, facilitating health education groups, and managing hospital admissions and discharges for Healthcare Home clients. You'll also play a crucial part in implementing cutting-edge health technology programs and monitoring performance measures using sophisticated information systems.

Key Responsibilities:

Develop and implement personalized treatment plans for Healthcare Home enrollees with co-occurring chronic diseasesFacilitate health education groups, utilizing advanced knowledge of complex medical conditionsManage hospital admissions and discharges for Healthcare Home clientsProvide education and training to patients on complex health conditions, medical diagnoses, treatments, and medicationsImplement and utilize health technology programs such as Cyberaccess, Care Manager, and IntellectAbilityMonitor and report performance measures and outcomes using sophisticated nursing information systemsCollaborate with multidisciplinary treatment teams to ensure comprehensive patient careDevelop and implement wellness and prevention initiativesParticipate in required meetings and quality assurance proceduresAdhere to Brightli's Operational Guidelines and professional code of ethics

Education, Experience, and/or Credential Qualifications:

Active RN or LPN licenseExperience with the IDD (Intellectual and Developmental Disabilities) population preferred

Additional Qualifications:

Knowledge of chronic disease processes in children and adultsExcellent written and verbal communication skillsAbility to implement innovative systems and make decisions based on critical thinking and nursing processSuccessful completion of background check including criminal record, driving record, and abuse/neglect screeningCurrent driver's license, acceptable driving record, and current auto insuranceMust complete New Hire Orientation at the beginning of employmentMust complete all training requirements including Relias at the beginning of employment and annually thereafter
