NURSE CASE MANAGER
The starting rate for this position is $35.10 an hour. The rate of pay displayed reflects the beginning of the pay scale. At the time of an offer, the determination of your offer will reflect your skills and experience as it relates to the position. Summary RN Case Managers utilize a systematic and collaborative approach to healthcare services for a defined population. The framework includes five components designed to enhance quality and cost-effective outcomes: assessment, planning, implementation, evaluation, and interaction to meet the individual patients’ health needs while decreasing the incidence of duplication of care. The RN Case Manager works closely with, and delegates care to, the High-Risk Social Worker /Behavioral Health Clinician after the assessment and determination of social needs. Job Requirements + Bachelor’s degree required. + Active State of Maine RN license. + Minimum of five years of broad clinical experience preferred. + Case Management and/or Utilization Manager experience preferred. + Certification as CCM or ACMA-RN preferred. + Proficient in Microsoft Office. + Strong verbal and written communication skills. + Strong interpersonal skills. + Ability to work within a stressful, fast-moving environment. + Knowledge of community agencies and resources preferred.
