Job Title: Nurse Case Manager

Job Description

Coordinate care for a select caseload of patients from triage, care coordination during management, and safe transfer to home. Proactively communicate with payors to ensure approval of treatment plans and required resources. Maintain knowledge of and respond to clinical system and fiscal data related to specific patient populations. Collaborate with the multi-disciplinary team, patient, and patient family to ensure effective and efficient transition planning. Coordinate transition to post-acute settings including but not limited to nursing facilities, acute inpatient rehab, and local shelters. Manage patients based on departmental procedures while maintaining clinical and quality standards. Enter and maintain patient information in all departmental systems per operational guidelines.

The position involves inpatient case management within the Oncology department. The nurse case manager will not have access to or pass medication to patients. The environment is dynamic and requires effective collaboration with a multidisciplinary team.

Pay and Benefits

The pay range for this position is $35.00 - $45.00/hr.
